IMPORTANT UPDATE | Patients of the below clinics can access medical records for care received after May 15, 2024, through their MyChart record or The University of Kansas Health System’s Health Information Management department. Learn more here. Medical records for care provided at the below clinics prior to May 15, 2024, can be accessed through an existing patient portal or one of the below avenues.
For Olathe Health Medical Records: Whether you want to receive digital access (patient portal or email delivery) or hard copies (faxed or mailed) of your medical records, patients and guardians can request copies of medical records following a visit at an Olathe Health facility.
Patient Portal
The patient portal offers free, online access to Olathe Health Physicians (OHP), Olathe Medical Center (OMC) and/or Miami County Medical Center (MCMC) health records. Here, you can see *much of your hospital or clinic visit information anytime it’s convenient for you.

Electronic Delivery (Email)
If you are a patient or guardian, you can request electronic access to complete medical records from OHP clinics, OMC or MCMC. These records will be sent in an electronic format by Ciox, our Release of Information Company.
(Note: This is not connected to your Patient Portal.)
Requesting Medical Records
Requests can be made two ways.
1. Online Request Form
Complete a secure online request form here.
Before you begin:
-Have a photo ID ready
-Have mobile phone nearby
-Use a Chrome, Firefox or Safari browser
2. Printed Request Form
–Print and complete the Authorization to Release Information Form
-Select your preferred delivery method on the Form (mail, fax, in-person pick up, eDelivery, other). If you are a patient or guardian, you can request electronic delivery of your medical records. These records will be sent in an electronic format by CIOX, our Release of Information company.
-Mail, fax or deliver in person to the OMC, MCMC address or fax number below.

Please note: You may be charged a cost-based fee. You may receive an invoice for these charges via mail from Ciox, the Release of Information company used by Olathe Health.
